The need for a new medical model: a challenge for biomedicine.

A biopsychosocial model is proposed that provides a blueprint for research, a framework for teaching, and a design for action in the real world of health care. Cognitive behavior therapy (CBT) has come to be a widely practiced psychotherapy throughout the world. The present article reviews theory, history, and evidence for CBT. It is meant as an effort to summarize the forms and scope of CBT to date for the uninitiated. Elements of CBT such as cognitive therapy, behavior therapy, and so-called "third wave" CBT, such as dialectical behavior therapy (DBT) and acceptance and commitment therapy (ACT) are covered. The evidence for the efficacy of CBT for various disorders is reviewed, including depression, anxiety disorders, personality disorders, eating disorders, substance abuse, schizophrenia, chronic pain, insomnia, and child/adolescent disorders. The relative efficacy of medication and CBT, or their combination, is also briefly considered. Future directions for research and treatment development are proposed. read more read less

The Effects of Intimate Partner Violence on Women and Child Survivors: An Attachment Perspective

Approximately 25% of women in the United States report having experienced intimate partner violence (IPV) in an adult relationship with a male partner. For affected women, IPV has been shown to increase the risk of psychopathology such as depression, anxiety, and symptoms of posttraumatic stress. Further, studies suggest that the risk of IPV (victimization or perpetration) may be carried intergenerationally, and children exposed to IPV are at a greater risk of both attachment insecurity and internalizing/externalizing problems. The authors employ an attachment perspective to describe how insecure/non-balanced working models of the relational self and others may be evoked by, elicit, or exacerbate maladaptive outcomes following experiences of IPV for mothers and their children. This article draws on both rich theory and empirical evidence in a discussion of attachment patterns in violent relationships, psychopathological outcomes for exposed women, disruptions in the caregiving relationship that may confer risk to children of exposed mothers, and the biological, social, and attachment risk factors for children exposed to IPV. A clinical case example is presented and discussed in the context of attachment theory. read more read less

Intervention to reduce traumatic stress following intimate partner violence: an efficacy trial of the Moms' Empowerment Program (MEP).
Sandra A. Graham-Bermann1, Laura E. Miller1
28 May 2013 - Psychodynamic psychiatry

Abstract:

A 10-week, group therapeutic-oriented community-based intervention, the Moms' Empowerment Program (MEP), was tested with 181 children and their mothers exposed to intimate partner violence (IPV) during the past year. A 10-week, group therapeutic-oriented community-based intervention, the Moms' Empowerment Program (MEP), was tested with 181 children and their mothers exposed to intimate partner violence (IPV) during the past year. Women's experiences of intimate partner violence and traumatic stress were assessed with standardized measures. A sequential assignment procedure allocated participants to three conditions: mother-plus-child intervention (M+C), child-only intervention (CO), and a wait list comparison (CG). The tensession group intervention program was designed to empower women thereby reducing traumatic stress and enhancing their coping in the aftermath of exposure to IPV. Eighty-one percent were diagnosed with PTSD. While women in all three conditions improved over time, those in the M+C condition showed the greatest reduction in traumatic stress as compared to CO or CG conditions. Of those M+C women diagnosed with PTSD, the reliable change rate was 85% no longer diagnosed. read more read less

The emerging evidence for long-term psychodynamic therapy.
Falk Leichsenring1, Allan Abbass, Patrick Luyten, Mark J. Hilsenroth, Sven Rabung
03 Sep 2013 - Psychodynamic psychiatry

Abstract:

There is growing evidence from RCTs supporting the efficacy of both short-term (STPP) and long-term psychodynamic psychotherapy (LTPP) for specific mental disorders. In a first series of meta-analyses, LTPP was shown to be superior to shorter forms of psychotherapy, especially in complex mental disorders. However, the evidenc... However, the evidence for LTPP has not gone unchallenged. After several responses have addressed the raised concerns, a recent meta-analysis by Smit and colleagues (2012) again challenges the efficacy of LTPP. Method: From a methodological perspective, a critical analysis of the Smit et al. meta-analysis was performed. Furthermore, we conducted two new metaanalyses adding studies not included in previous meta-analyses. The purpose was to examine whether the results of the previous meta-analyses are stable. Results: Due to differing inclusion criteria, the meta-analysis by Smit et al. actually compared LTPP to other forms of long-term psychotherapy. Thus, they essentially showed that LTPP was as ... read more read less
