Introdução ao Pensamento Complexo

The need for critical thinking in nursing has been accentuated in response to the rapidly changing health care environment. Nurses must think critically to provide effective care whilst coping with the expansion in role associated with the complexities of current health care systems. This literature review will present a history of inquiry into critical thinking and research to support the conclusion that critical thinking is necessary not only in the clinical practice setting, but also as an integral component of nursing education programs to promote the development of nurses’ critical thinking abilities. The aims of this paper are: (a) to review the literature on critical thinking; (b) to examine the dimensions of critical thinking; (c) to investigate the various critical thinking strategies for their appropriateness to enhance critical thinking in nurses, and; (d) to examine issues relating to evaluation of critical thinking skills in nursing.

The scope of this paper is to take a retrospective look at the professional role of the nurse in the Brazilian Unified Health System and understand the meaning of their social practice in this field of discussions and theoretical-practical meanings. The Grounded Theory was used as a methodological reference and interviews were the technique for data collection, conducted between May and December of 2007, with 35 health professionals and others. The codification and data analysis revealed that the Unified Health System and especially the Family Health Strategy, should be considered facilitating and stimulating strategies of the process of expansion and consolidation of nursing care as a social entrepreneurial practice, since they pave the way to a new approach in community intervention, through seeing the human being as a multi-dimensional and singular being, inserted in a real and concrete context.

Estudo qualitativo, partiu da questao: como se da o cuidado intensivo oferecido ao paciente no ambiente de Unidade de Terapia Intensiva? Teve como objetivo compreender o cuidado intensivo oferecido ao paciente no ambiente de Unidade de Terapia Intensiva Adulto. O metodo utilizado foi a Grounded Theory. Realizou-se 39 entrevistas com 47 sujeitos diferenciados (pacientes, profissionais da saude, gestores, profissionais dos servicos de apoio, familiares) de tres Unidades de Terapia Intensiva Adulto, em Florianopolis/SC, Santa Maria/RS e Pelotas/RS, Brasil, entre junho de 2009 a setembro de 2010. A teoria \"Sustentando a vida no ambiente complexo de cuidados em Unidade de Terapia Intensiva\", foi delimitada por oito categorias, das quais sera focalizada neste artigo a categoria \"Cuidando e monitorando o paciente continuamente\". Conclui-se que o cuidado intensivo requer um cuidado integral com os pacientes, que vai alem do cuidado tecnico e inclui as dimensoes fisica, emocional, espiritual e social do ser humano.

El cuidado intensivo ofrecido al paciente en el ambiente de la Unidad de Cuidados Intensivos

Estudo qualitativo que partiu da questao: como vem sendo construidas as teorias e modelos de cuidado de enfermagem, focalizando o processo de construcao da teoria substantiva, referente a Tese que teve como objetivos compreender o significado do ambiente de cuidados em Unidade de Terapia Intensiva e construir um modelo teorico sobre ele. O metodo utilizado foi a Grounded Theory. Realizaram-se 39 entrevistas com 47 sujeitos diferenciados de tres Unidades de Terapia Intensiva Adulto, em Florianopolis/SC, Santa Maria/RS e Pelotas/RS, entre junho de 2009 a setembro de 2010. A teoria "Sustentando a vida no ambiente complexo de cuidados em Unidade de Terapia Intensiva" foi delimitada por oito categorias. Conclui-se que sustentar a vida no ambiente de Unidade de Terapia Intensiva significa investir intensivamente no cuidado de pacientes instaveis, com auxilio de tecnologias diferenciadas e profissionais capacitados, trabalhando em equipe, onde se convive com estresse/conflitos e dificuldades para lidar com a morte.

Desenvolvimento e validação de teoria fundamentada em dados sobre o ambiente de unidade de terapia intensiva

Background: Informal caregiving by family members is the most common way of caring for sick people at home. However, the number of care arrangements, in which both formal (nurses) and informal (family members) caregivers are involved, is considerable and increasing. Despite implicit assumptions in research that the involvement of nurses in home care arrangements is inherently beneficial, there is evidence that their involvement may have a destabilising effect.

Aims: The purpose of this study was to investigate the relationship between nurses and family caregivers and its impact on the actual care that is provided.

Method: Eighty-eight interviews with family caregivers (n = 57) and nurses (n = 31) were conducted in Germany and analysed according to the Grounded Theory methodology.

Findings: The relationship between formal and informal care is an encounter of two quite different perspectives that is focused on a negotiation process about caregiving work and the helpfulness of the actions taken and the interventions used. For family caregivers, it is determined by the goal of facilitating work and care for their sick family member. The nurses’ work is characterised by a process of shaping different realities in different homes. The results reveal the processes that lead to the involvement of nurses into home care arrangements and offer a deeper understanding of the negotiation processes between formal and informal caregivers.

Conclusions: To provide sufficient support in home care, nurses need the ability to engage in negotiation processes that take the whole home care arrangement into account. Developmental work is needed to design services that are helpful for family caregivers.
