Tuberculosis-associated immune reconstitution inflammatory syndrome: case definitions for use in resource-limited settings.
Graeme Meintjes,Stephen D. Lawn,Stephen D. Lawn,Fabio Scano,Gary Maartens,Martyn A. French,William Worodria,Julian Elliott,David M Murdoch,Robert J. Wilkinson,Robert J. Wilkinson,Robert J. Wilkinson,Catherine Seyler,Laurence John,Maarten F. Schim van der Loeff,Peter Reiss,Lut Lynen,Edward N. Janoff,Charles F. Gilks,Robert Colebunders,Robert Colebunders +20 more
TL;DR: In this paper, consensus case definitions for paradoxical tuberculosis-associated IRIS, ART-associated tuberculosis, and unmasking tuberculosis associated IRIS were derived, which can be used by clinicians and researchers in a variety of settings to promote standardisation and comparability of data.
Journal ArticleDOI
Timing of antiretroviral therapy after diagnosis of cryptococcal meningitis
David R. Boulware,David B. Meya,David B. Meya,Conrad Muzoora,Melissa A. Rolfes,Katherine Huppler Hullsiek,Abdu K Musubire,Kabanda Taseera,Henry W. Nabeta,Charlotte Schutz,Darlisha A. Manabe,Edward N. Janoff,Edward N. Janoff,Paul R. Bohjanen,Graeme Meintjes,Graeme Meintjes +26 more
TL;DR: Deferring ART for 5 weeks after the diagnosis of cryptococcal meningitis was associated with significantly improved survival, as compared with initiating ART at 1 to 2 weeks, especially among patients with a paucity of white cells in cerebrospinal fluid.
Journal ArticleDOI
Randomized placebo-controlled trial of prednisone for paradoxical tuberculosis-associated immune reconstitution inflammatory syndrome
Graeme Meintjes,Robert J. Wilkinson,Chelsea Morroni,Dominique J. Pepper,Kevin Rebe,Molebogeng X Rangaka,Tolu Oni,Gary Maartens +7 more
TL;DR: Prednisone reduced the need for hospitalization and therapeutic procedures and hastened improvements in symptoms, performance, and quality of life while investigating for drug-resistant tuberculosis and other causes for deterioration before administering glucocorticoids.
Journal ArticleDOI
Determinants of Mortality in a Combined Cohort of 501 Patients With HIV-Associated Cryptococcal Meningitis: Implications for Improving Outcomes
Joseph N Jarvis,Tihana Bicanic,Angela Loyse,Daniel Namarika,Arthur Jackson,Jesse C. Nussbaum,Jesse C. Nussbaum,Nicky Longley,Nicky Longley,Conrad Muzoora,Jacob Phulusa,Kabanda Taseera,Creto Kanyembe,Douglas Wilson,Mina C. Hosseinipour,Annemarie E. Brouwer,Direk Limmathurotsakul,Nicholas J. White,Nicholas J. White,Charles van der Horst,Robin Wood,Graeme Meintjes,Graeme Meintjes,John S. Bradley,Shabbar Jaffar,Thomas S. Harrison +25 more
TL;DR: Cerebrospinal fluid fungal burden, altered mental status, and rate of clearance of infection predict acute mortality in HIV-associated cryptococcal meningitis.
Journal ArticleDOI
Evaluation of a Novel Point-of-Care Cryptococcal Antigen Test on Serum, Plasma, and Urine From Patients With HIV-Associated Cryptococcal Meningitis
Joseph N Jarvis,Ann Percival,Sean K. Bauman,Joy M. Pelfrey,Graeme Meintjes,Graeme Meintjes,GN Williams,Nicky Longley,Nicky Longley,Thomas S. Harrison,Thomas R. Kozel +10 more
TL;DR: This novel dipstick test has the potential to markedly improve early diagnosis of CM in many settings, enabling testing of urine in patients presenting to health care facilities in which lumbar puncture, or even blood sampling, is not feasible.
