Abstract: Pressure and flow were recorded in the ascending aorta of three dogs with aortic coarctation, induced surgically 3 months previously, and in three control litter mates. From these data were derived input impedance of the systemic circulation, pulsatile and steady components of external heart work, aortic systolic pressure-time index, and mean systolic and mean diastolic pressures. Results were correlated with intraoperative measurements of aortic pressure in three young patients with aortic coarctation and with records taken in another 24 patients during diagnostic catheterization. Distinctive changes in aortic impedance with coarctation were explained in terms of altered peripheral reflection. Such changes were responsible for characteristic alteration in central aortic pressure pulse contour and for change in other parameters, indicating impaired arterial function in accepting pulsatile flow from the heart. Such changes in the cushioning function appear responsible for many clinical features and complications of aortic coarctation. Surgery is probably justified for restoration of the normal cushioning function of the arterial system as well as for reduction of resistance and mean arterial pressure.

Abstract: Summary. Morphological features of the peripheral blood of the ancestral elasmo-branch Heterodontus portusjacksoni, a shark species, are described. The red cell is nucleated and contains many membrane-lined vacuoles which are derived from mitochondria. The granulocytic series resembles that of the mammals in appearance and in some phagocytic properties. The thrombocytes possess a number of similarities to mammalian platelets, but exhibit many differences. They aggregate readily, but this is temperature reversible and is independent of thrombin and ADP. They show a remarkable affinity for latex particles. Unlike many other species, fibrin formation and thrombocyte aggregation are apparently independent phenomena occurring in parallel rather than in series. Tritiated thymidine studies and the observation of mitotic figures in the peripheral blood suggest that at least part of the red cell and thrombocyte proliferation occurs in the circulating blood.

Abstract: A simple, rapid, and accurate method is described for determining diphenylhydantoin concentrations in plasma. An internal standard, primidone (primaclone), is added to 2 ml of acidified plasma, and primidone and diphenylhydantoin are extracted into chloroform. The extract, washed with hexane and concentrated, is injected into a gas chromatograph. Extraction and measurement of a single sample requires about 30 min, and as many as 40 samples can be assayed in a working day. Both therapeutic and overdose concentrations of diphenylhydantoin in plasma can be detected and measured. Nine other acidic drugs, added to plasma samples, did not interfere with the analysis.

Abstract: Summary Twelve patients with ventricular aneurysm were studied and of these six were treated by resection without mortality. Ten of the cases followed clear evidence of myocardial infarction but in two no definite history of myocardial infarction was elicited. Clinical features commonly observed were refractory congestive cardiac failure, recurrent ventricular arrhythmia or severe angina pectoris. The cases illustrate that a surgically remediable ventricular aneurysm may be present in the absence of classical ECG or plain X-ray abnormalities and demonstrate the importance of cineventriculo-graphy in the diagnosis. The results indicate the importance of considering ventricular aneurysm in patients with coronary artery disease and resistant heart failure, angina or arrhythmia.

Abstract: A trial of chlorambucil has been undertaken at the Royal Prince Alfred Hospital over the last two years in the treatment of severe rheumatoid arthritis. Two groups of patients have been followed: (i) patients with severe, unremitting rheumatoid arthritis who had been on steroid therapy for long periods; (ii) eight patients with fulminant rheumatoid arthritis who had not previously been on steroids.

Abstract: Summary: The clinical and immunological features of 2 cases of γD myeloma are compared with other reported cases of γD myeloma and other types of multiple myeloma. Both cases showed extra-medullary plasmacytoma which has recently been described as a frequent finding in γD myeloma but which is unusual in other types of multiple myeloma. Bence Jones proteinuria was present in both cases. This is in line with the higher incidence in γD myeloma compared with other types of multiple myeloma. Immunoelectrophoresis showed one case with the more typical L type light chain, the other with the less common K type. Serum levels of the γD paraprotein were low in both instances, reflecting the higher catabolic rate of γD immunoglobulin.

Abstract: A Doppler ultrasonic flow detector has been used to determine the incidence of deep‐vein thrombosis in a general surgical ward. The incidence was shown to be 30.6% among all patients whether they came to surgery or not. The age at greatest risk was between 60 and 70 years, but the patients suffering thromboses were not significantly older than the remainder. Cardio‐vascular disease, active inflammatory states, biliary disease, endocrine disorders (particularly diabetes), metabolic disorders demonstrated by standard biochemical screening tests, obesity, and hæmorrhage requiring transfusion were all noted among these patients, but no single disorder appeared to carry a significantly higher risk of venous thrombosis. Abdominal operations and those involving the legs carried high risk. Patients who stayed more than 20 days in hospital had a highly significant increase in the incidence of venous thrombosis, and it is suggested that long‐stay patients may constitute an especially high risk group. Only eight of the 37 thrombotic episodes were detected clinically. Among the five patients in whom pulmonary emboli were detected, only one had clinically obvious disease. in the other patients, the venous thrombosis was diagnosed by the Doppler flow detector, either before the embolic episode or at the time of its occurrence.
