Abstract: BACKGROUND: Consensus guidelines on the definition, investigation, and treatment of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) have been previously published in European Journal of Neurology and Journal of the Peripheral Nervous System. OBJECTIVES: To revise these guidelines. METHODS: Disease experts, including a representative of patients, considered references retrieved from MEDLINE and Cochrane Systematic Reviews published between August 2004 and July 2009 and prepared statements that were agreed in an iterative fashion. RECOMMENDATIONS: The Task Force agreed on Good Practice Points to define clinical and electrophysiological diagnostic criteria for CIDP with or without concomitant diseases and investigations to be considered. The principal treatment recommendations were: (i) intravenous immunoglobulin (IVIg) (Recommendation Level A) or corticosteroids (Recommendation Level C) should be considered in sensory and motor CIDP; (ii) IVIg should be considered as the initial treatment in pure motor CIDP (Good Practice Point); (iii) if IVIg and corticosteroids are ineffective, plasma exchange (PE) should be considered (Recommendation Level A); (iv) if the response is inadequate or the maintenance doses of the initial treatment are high, combination treatments or adding an immunosuppressant or immunomodulatory drug should be considered (Good Practice Point); (v) symptomatic treatment and multidisciplinary management should be considered (Good Practice Point).

Abstract: Improved glucose control in type 2 diabetes is known to reduce the risk of microvascular events. There is, however, continuing uncertainty about its impact on macrovascular disease. The aim of these analyses was to generate more precise estimates of the effects of more-intensive, compared with less-intensive, glucose control on the risk of major cardiovascular events amongst patients with type 2 diabetes. A prospectively planned group-level meta-analysis in which characteristics of trials to be included, outcomes of interest, analyses and subgroup definitions were all pre-specified. A total of 27,049 participants and 2,370 major vascular events contributed to the meta-analyses. Allocation to more-intensive, compared with less-intensive, glucose control reduced the risk of major cardiovascular events by 9% (HR 0.91, 95% CI 0.84–0.99), primarily because of a 15% reduced risk of myocardial infarction (HR 0.85, 95% CI 0.76–0.94). Mortality was not decreased, with non-significant HRs of 1.04 for all-cause mortality (95% CI 0.90–1.20) and 1.10 for cardiovascular death (95% CI 0.84–1.42). Intensively treated participants had significantly more major hypoglycaemic events (HR 2.48, 95% CI 1.91–3.21). Exploratory subgroup analyses suggested the possibility of a differential effect for major cardiovascular events in participants with and without macrovascular disease (HR 1.00, 95% CI 0.89–1.13, vs HR 0.84, 95% CI 0.74–0.94, respectively; interaction p = 0.04). Targeting more-intensive glucose lowering modestly reduced major macrovascular events and increased major hypoglycaemia over 4.4 years in persons with type 2 diabetes. The analyses suggest that glucose-lowering regimens should be tailored to the individual.

Abstract: This paper studies unmanned aerial vehicle (UAV)-enabled wireless communication, where a rotary-wing UAV is dispatched to communicate with multiple ground nodes (GNs). We aim to minimize the total UAV energy consumption, including both propulsion energy and communication related energy, while satisfying the communication throughput requirement of each GN. To this end, we first derive a closed-form propulsion power consumption model for rotary-wing UAVs, and then formulate the energy minimization problem by jointly optimizing the UAV trajectory and communication time allocation among GNs, as well as the total mission completion time. The problem is difficult to be optimally solved, as it is non-convex and involves infinitely many variables over time. To tackle this problem, we first consider the simple fly-hover-communicate design, where the UAV successively visits a set of hovering locations and communicates with one corresponding GN while hovering at each location. For this design, we propose an efficient algorithm to optimize the hovering locations and durations, as well as the flying trajectory connecting these hovering locations, by leveraging the travelling salesman problem with neighborhood and convex optimization techniques. Next, we consider the general case, where the UAV also communicates while flying. We propose a new path discretization method to transform the original problem into a discretized equivalent with a finite number of optimization variables, for which we obtain a high-quality suboptimal solution by applying the successive convex approximation technique. The numerical results show that the proposed designs significantly outperform the benchmark schemes.
