Variable skin pH values are being reported in literature, all in the acidic range but with a broad range from pH 4.0 to 7.0. In a multicentre study (N = 330), we have assessed the skin surface pH of the volar forearm before and after refraining from showering and cosmetic product application for 24 h. The average pH dropped from 5.12 +/- 0.56 to 4.93 +/- 0.45. On the basis of this pH drop, it is estimated that the 'natural' skin surface pH is on average 4.7, i.e. below 5. This is in line with existing literature, where a relatively large number of reports (c. 50%) actually describes pH values below 5.0; this is in contrast to the general assumption, that skin surface pH is on average between 5.0 and 6.0. Not only prior use of cosmetic products, especially soaps, have profound influence on skin surface pH, but the use of plain tap water, in Europe with a pH value generally around 8.0, will increase skin pH up to 6 h after application before returning to its 'natural' value of on average below 5.0. It is demonstrated that skin with pH values below 5.0 is in a better condition than skin with pH values above 5.0, as shown by measuring the biophysical parameters of barrier function, moisturization and scaling. The effect of pH on adhesion of resident skin microflora was also assessed; an acid skin pH (4-4.5) keeps the resident bacterial flora attached to the skin, whereas an alkaline pH (8-9) promotes the dispersal from the skin.

Dynamic optical coherence elastography is used to determine in vivo skin biomechanical properties based on mechanical surface wave propagation. Quantitative Young's moduli are measured on human skin from different sites, orientations, and frequencies. Skin thicknesses, including measurements from different layers, are also measured simultaneously. Experimental results show significant differences among measurements from different skin sites, between directions parallel and orthogonal to Langer's lines, and under different skin hydration states. Results also suggest surface waves with different driving frequencies represent skin biomechanical properties from different layers in depth. With features such as micrometer-scale resolution, noninvasive imaging, and real-time processing from the optical coherence tomography technology, this optical measurement technique has great potential for measuring skin biomechanical properties in dermatology.

Biomechanical Properties of In Vivo Human Skin From Dynamic Optical Coherence Elastography

Mechanical assessment of soft biological tissues and organs has broad relevance in clinical diagnosis and treatment of disease. Existing characterization methods are invasive, lack microscale spatial resolution, and are tailored only for specific regions of the body under quasi-static conditions. Here, we develop conformal and piezoelectric devices that enable in vivo measurements of soft tissue viscoelasticity in the near-surface regions of the epidermis. These systems achieve conformal contact with the underlying complex topography and texture of the targeted skin, as well as other organ surfaces, under both quasi-static and dynamic conditions. Experimental and theoretical characterization of the responses of piezoelectric actuator-sensor pairs laminated on a variety of soft biological tissues and organ systems in animal models provide information on the operation of the devices. Studies on human subjects establish the clinical significance of these devices for rapid and non-invasive characterization of skin mechanical properties.

Conformal piezoelectric systems for clinical and experimental characterization of soft tissue biomechanics.

BACKGROUND/AIMS: A new method for the in vivo characterization of the mechanical properties of skin has been developed. This comprises a suction chamber and an ultrasound device to measure both the vertical displacement of the skin's surface, and the skin's thickness. METHODS: A mathematical model of the mechanical behaviour of a taught elastic membrane is used to obtain a set of parameters intrinsic to the skin, such as Young's modulus (E) and the initial stress (sigma0), which reflect the stiffness and the natural tension of the skin, respectively. We also calculated an index of non-elasticity of the skin (unrestored energy ratio, UER), which takes into account the volume of tissue mobilized. It determines a ratio between the energy input to the skin and the energy it dissipates. These parameters were evaluated from the volar forearm of 10 normal male volunteers. RESULTS: The results were: 129+/-88 kPa for E, 13.5+/-5 kPa for sigma0, and 0.42+/-0.04 for UER; with reproducibilities of 9.5%, 12.4% and 6.4%, respectively. CONCLUSIONS: This new suction device was found useful for the study of the behaviour of the skin, and the device may be used for the evaluation of certain skin diseases and their therapy.

In vivo model of the mechanical properties of the human skin under suction.

BACKGROUND/AIMS: Two types of skin reflectance instruments are available nowadays for the determination of skin color: a tristumulus colorimeter (Chromameter from Minolta) using the CIE L*a*b* color system and the narrow-band simple reflectance meters (DermaSpectrometer from Cortex and Mexameter from Courage-Khazaka) using the erythema/melanin indices. The purpose of this study was to compare the capabilities of the three instruments (sensitivity, repeatability and correlation) in vitro and in vivo. METHODS: Comparative color measurements were carried out first in vitro on standardized color charts and subsequently in vivo on different skin areas in human volunteers. Skin color changes induced by various physico-chemical treatments were also quantitatively evaluated with the three instruments. RESULTS: The in vitro and in vivo repeatabilty as well as the sensitivity of the three instruments are rather good. Erythema and skin blanching could be readily quantified by the increase of the a* parameter and of the erythema indices of the simple reflectance meters. Natural UV tanning and artificial chemical tanning could be measured by the decrease of L* and increase of b* and of the melanin indices. CONCLUSION: The Chromameter and the two narrow-band reflectance instruments were able to characterize skin color and to quantify small skin color changes. Moderate to high significant linear correlations could be established between the CIE L*a*b* color parameters and the erythema/melanin indices.

Background/aims: The skin irritation potential of a body cleansing product is often compared under exaggerated test conditions, although the product is intended to be used at home with repetitive and brief contact with the skin. The aim of this study was to determine how much patch testing is predictive of the clinical, sub-clinical and subjective cutaneous effects of products used at home by consumers for their normal hygienic cleansing. Methods: A double-blind comparative study of the normal use of an alkaline soap bar and a syndet at home during 10 consecutive weeks was performed on two identical groups of 25 healthy female subjects. The eventual skin changes observed at different anatomical skin sites were evaluated by clinical visual examination and by bioengineering measurements before the start of the study and then every 2 weeks. The objective measurements were compared with the subject’s perceptions of dryness, tightness and product irritancy during the testing.
