The World Health Organization(WHO) has recently published a comprehensive report entitled Adherence to Long-Term Therapies: Evidence for Action w1x. This report focuses on nine chronic conditions and their riskfactors. The conditions reviewed are hypertension, tobacco smoking cessation, asthma, cancer (palliative care), depre... The World Health Organization(WHO) has recently published a comprehensive report entitled Adherence to Long-Term Therapies: Evidence for Action w1x. This report focuses on nine chronic conditions and their riskfactors. The conditions reviewed are hypertension, tobacco smoking cessation, asthma, cancer (palliative care), depression, diabetes, epilepsy, HIV yAIDS and tuberculosis. The report reviewed the available literature on the epidemiology of nonadherence worldwide; it stressed the multi-factorial nature of nonadherence, it identified health systems and health care teams as significant determinants to good adherence and discussed the health and economic consequences of nonadherence as well as some strategies for improving it. This report has major relevance for cardiovascular nurses. Cardiovascular disease is and will remain a major challenge for health care professionals and health care systems, and it contributes significantly to the global burden of disease in both developed and developing countriesw2x. Nonadherence to treatment recommendations in patients with cardiovascular risk-factors as well as patients with cardiovascular disease is widespread, and it is a major factor contributing to poor outcome. Nurses are excellently positioned to target the behavioral dimension of chronic disease management. They can improve outcomes by developing and implementing adherence-enhancing strategies to reduce cardiovascular risk-factors, and to enhance adherence with non-smoking, diet, exercise and medication regimens w3x. The value of this WHO report is not only that it comprehensively reviews the current state of the literature including hypertension and tobacco smoking cessation, but also it highlights the need for a multi-disciplinary approach to adherence, emphasizes system factors that need to be addressed in successfully implementing adherence-enhancing strategies. Moreover, it also provides illustrative examples of the ways in which professionals have contributed in their own fields of expertise including cardiovascular care among other fields. This report, therefore, provides an instrument to cardiovascular nurses to expand the behavioral dimension of their patient management strategies, to teach students at undergraduate and graduate level, to guide their research endeavors, to guide policy makers, ultimately with the goal to improve outcomes of populations. read more read less

Person-centered care (PCC) has been shown to advance concordance between care provider and patient on treatment plans, improve health outcomes and increase patient satisfaction. Yet, despite these and other documented benefits, there are a variety of significant challenges to putting PCC into clinical practice. Although care providers today broadly acknowledge PCC to be an important part of care, in our experience we must establish routines that initiate, integrate, and safeguard PCC in daily clinical practice to ensure that PCC is systematically and consistently practiced, i.e. not just when we feel we have time for it. In this paper, we propose a few simple routines to facilitate and safeguard the transition to PCC. We believe that if conscientiously and systematically applied, they will help to make PCC the focus and mainstay of care in long-term illness. read more read less

Despite significant scientific advances, heart failure patients experience multiple physical and psychological symptoms that can impact the quality of life.Aims: To determine the (1) symptom prevalence, severity, distress and symptom burden in patients with heart failure; (2) impact of age and gender on symptom prevalence, severity, distress and symptom burden; and (3) impact of symptom prevalence and symptom burden on health-related quality of life (HRQOL) in patients with heart failure.Methods: A convenience sample of 53 heart failure patients participated in this descriptive, cross-sectional design. Symptoms and HRQOL were measured using the Memorial Symptom Assessment Scale—Heart Failure and the Minnesota Living with Heart Failure Questionnaire.Results: Patients experienced a mean of 15.1 ± 8.0 symptoms. Platelets play an important, but often under-recognized role in cardiovascular disease. For example, the normal response of the platelet can be altered, either by increased pro-aggregatory stimuli or by diminished anti-aggregatory substances to produce conditions of increased platelet activation/aggregation and occur in active cardiovascular disease states both on a chronic (e.g. stable angina pectoris) and acute basis (e.g. acute myocardial infarction). In addition, platelet hyperaggregability is also associated with the risk factors for coronary artery disease (e.g. smoking, hypertension, and hypercholesterolaemia). Finally, the utility of an increasing range of anti-platelet therapies in the management of the above disease states further emphasizes the pivotal role platelets play in the pathogenesis of cardiovascular disease. This paper provides a comprehensive overview of the normal physiologic role of platelets in maintain homeostasis, the pathophysiologic processes that contribute to platelet dysfunction in cardiovascular disease and the associated role and benefits of anti-platelet therapies. read more read less

Consequently, the suffering in patients with end-stage CHF is still present. The knowledge on the frequency of symptoms in these patients is scarce. Aims: The aim of this study was to achieve a deeper understanding of patients with end-stage CHF and to describe symptoms in these patients during the last 6 months of life by examining documentation in medical records. Methods: The study adopted a descriptive, retrospective design using record reviews to collect data, in which 80 medical records of patients hospitalised for CHF in 1995 were reviewed. Descriptive statistics and Student's t-tests were performed. Results: Breathlessness was the most common symptom (men 90%, women 86%).
