Developments, application, and performance of artificial intelligence in dentistry - A systematic review.

To evaluate the performance of a new artificial intelligence (AI)-driven tool for tooth detection and segmentation on panoramic radiographs. In total, 153 radiographs were collected. A dentomaxillofacial radiologist labeled and segmented each tooth, serving as the ground truth. Class-agnostic crops with one tooth resulted in 3576 training teeth. The AI-driven tool combined two deep convolutional neural networks with expert refinement. Accuracy of the system to detect and segment teeth was the primary outcome, time analysis secondary. The Kruskal-Wallis test was used to evaluate differences of performance metrics among teeth groups and different devices and chi-square test to verify associations among the amount of corrections, presence of false positive and false negative, and crown and root parts of teeth with potential AI misinterpretations. The system achieved a sensitivity of 98.9% and a precision of 99.6% for tooth detection. For segmenting teeth, lower canines presented best results with the following values for intersection over union, precision, recall, F1-score, and Hausdorff distances: 95.3%, 96.9%, 98.3%, 97.5%, and 7.9, respectively. Although still above 90%, segmentation results for both upper and lower molars were somewhat lower. The method showed a clinically significant reduction of 67% of the time consumed for the manual. The AI tool yielded a highly accurate and fast performance for detecting and segmenting teeth, faster than the ground truth alone. An innovative clinical AI-driven tool showed a faster and more accurate performance to detect and segment teeth on panoramic radiographs compared with manual segmentation.

Artificial intelligence-driven novel tool for tooth detection and segmentation on panoramic radiographs

The aim of this study was to evaluate the success of the artificial intelligence (AI) system in implant planning using three-dimensional cone-beam computed tomography (CBCT) images. Seventy-five CBCT images were included in this study. In these images, bone height and thickness in 508 regions where implants were required were measured by a human observer with manual assessment method using InvivoDental 6.0 (Anatomage Inc. San Jose, CA, USA). Also, canals/sinuses/fossae associated with alveolar bones and missing tooth regions were detected. Following, all evaluations were repeated using the deep convolutional neural network (Diagnocat, Inc., San Francisco, USA) The jaws were separated as mandible/maxilla and each jaw was grouped as anterior/premolar/molar teeth region. The data obtained from manual assessment and AI methods were compared using Bland–Altman analysis and Wilcoxon signed rank test. In the bone height measurements, there were no statistically significant differences between AI and manual measurements in the premolar region of mandible and the premolar and molar regions of the maxilla (p > 0.05). In the bone thickness measurements, there were statistically significant differences between AI and manual measurements in all regions of maxilla and mandible (p < 0.001). Also, the percentage of right detection was 72.2% for canals, 66.4% for sinuses/fossae and 95.3% for missing tooth regions. Development of AI systems and their using in future for implant planning will both facilitate the work of physicians and will be a support mechanism in implantology practice to physicians.

A deep learning approach for dental implant planning in cone-beam computed tomography images.

Objective This investigation aimed to verify and compare the performance of 3 deep learning systems for classifying maxillary impacted supernumerary teeth (ISTs) in patients with fully erupted incisors. Study Design In total, the study included 550 panoramic radiographs obtained from 275 patients with at least 1 IST and 275 patients without ISTs in the maxillary incisor region. Three learning models were created by using AlexNet, VGG-16, and DetectNet. Four hundred images were randomly selected as training data, and 100 images were assigned as validating and testing data. The remaining 50 images were used as new testing data. The sensitivity, specificity, accuracy, and area under the receiver operating characteristic curve were calculated. Detection performance was evaluated by using recall, precision, and F-measure. Results DetectNet generally produced the highest values of diagnostic efficacy. VGG-16 yielded significantly lower values compared with DetectNet and AlexNet. Assessment of the detection performance of DetectNet showed that recall, precision, and F-measure for detection in the incisor region were all 1.0, indicating perfect detection. Conclusions DetectNet and AlexNet appear to have potential use in classifying the presence of ISTs in the maxillary incisor region on panoramic radiographs. Additionally, DetectNet would be suitable for automatic detection of this abnormality.

The aim of this study was to evaluate the use of a convolutional neural network (CNN) system for detecting vertical root fracture (VRF) on panoramic radiography. Three hundred panoramic images containing a total of 330 VRF teeth with clearly visible fracture lines were selected from our hospital imaging database. Confirmation of VRF lines was performed by two radiologists and one endodontist. Eighty percent (240 images) of the 300 images were assigned to a training set and 20% (60 images) to a test set. A CNN-based deep learning model for the detection of VRFs was built using DetectNet with DIGITS version 5.0. To defend test data selection bias and increase reliability, fivefold cross-validation was performed. Diagnostic performance was evaluated using recall, precision, and F measure. Of the 330 VRFs, 267 were detected. Twenty teeth without fractures were falsely detected. Recall was 0.75, precision 0.93, and F measure 0.83. The CNN learning model has shown promise as a tool to detect VRFs on panoramic images and to function as a CAD tool.

Dental record plays an important role in dental diagnosis and personal identification. Automatic image preinterpretation can help reducing dentists’ workload and improving diagnostic efficiency. Systematic dental record filing enables effective utilization of accumulated records at dental clinics for forensic identification. We have been investigating a tooth labeling method on dental cone-beam CT images for the purpose of automatic filing of dental charts. In our previous method, two separate networks were employed for detection and classification of teeth. Although detection accuracy was promising, classification performance had a room of improvement. The purpose of this study was to investigate the use of the relation network to utilize information of positional relationship between teeth for the detection and classification. Using the proposed method, both detection and classification performance improved. Especially, the tooth type classification accuracy improved. The proposed method can be useful in automatic filing of the dental chart.
