Novel insights into the biology of myelodysplastic syndromes: excessive apoptosis and the role of cytokines.
Azra Raza,Suneel D. Mundle,Vilasini Shetty,Sairah Alvi,H. Chopra,Lambert F.R. Span,Agapi Parcharidou,Saleem Dar,Parameswaran Venugopal,R. Z. Borok,Sefer Gezer,J.L. Showel,Jerome Loew,E. Robin,S. Rifkin,D. Alston,B. Hernandez,R. Shah,H. Kaizer,Stephanie A. Gregory +19 more
TL;DR: A dual role for TNF-alpha (or other cytokines) in the pathogenesis of MDS is proposed, where on the one hand, T NF-alpha induces apoptosis in the maturing cells causing pancytopenia while on the other, it stimulates the proliferation of the primitive progenitors accounting for the hypercellular BM frequently seen in MDS.
Journal ArticleDOI
Adrenocortical function: an indicator of severity of disease and survival in chronic critically ill patients.
Lambert F.R. Span,A. R. M. M. Hermus,A. K. M. Bartelink,Andries J. Hoitsma,J. S. F. Gimbrère,A. G. H. Smals,Peter W. C. Kloppenborg +6 more
TL;DR: It is suggested that a high plasma cortisol level, like a high Apache IIm-stay score, indicates severity of disease and poor survival in critically ill patients.
Journal ArticleDOI
Plasma and Whole Blood Exchange in Meningococcal Sepsis
M. van Deuren,Frederik W Santman,R. van Dalen,Robert W. Sauerwein,Lambert F.R. Span,J.W.M. van der Meer +5 more
TL;DR: It is concluded that early initiation of PEBE may improve the rate of survival among patients with meningococcal infection and hypotension but that the mechanism of the beneficial effect is most likely not based on the elimination of endotoxin.
Journal ArticleDOI
PCR-based detection of Aspergillus fumigatus Cyp51A mutations on bronchoalveolar lavage: a multicentre validation of the AsperGenius assay® in 201 patients with haematological disease suspected for invasive aspergillosis.
G. M. Chong,M.T. van der Beek,P.A. von dem Borne,Jerina Boelens,Eva Steel,Greetje A. Kampinga,Lambert F.R. Span,Katrien Lagrou,J Maertens,Gijs Dingemans,Giel Gaajetaan,D. W. E. van Tegelen,Jan J. Cornelissen,Alieke G Vonk,Bart J. A. Rijnders +14 more
TL;DR: The AsperGenius® assay had a good diagnostic performance on BAL and differentiated WT from Aspergillus fumigatus with RAMs, including in culture-negative BAL samples, and detection of RAMs was associated with azole treatment failure.
Journal ArticleDOI
Therapeutic Drug Monitoring of Posaconazole: an Update
Bart G. J. Dekkers,Martijn Bakker,Kim C. M. van der Elst,Marieke G G Sturkenboom,Anette Veringa,Lambert F.R. Span,Jan-Willem C. Alffenaar +6 more
TL;DR: The most recent findings on pharmacokinetics and pharmacodynamics of posaconazole in the setting of prophylaxis and treatment of fungal infections are highlighted and the challenges associated with TDM are referred to.
