Abstract: Background Generally, conservative interventions including physiotherapy modalities and manual therapy have been recommended in the management of carpal tunnel syndrome (CTS), but this subject has not been studied in diabetic patients with CTS. Therefore the aim of this study was to investigate the effects of manual therapy on diabetic patients with CTS. Methods Thirty diabetic patients with CTS were randomly divided into two equal groups: modality group and manual therapy group. Participants in the modality group received transcutaneous electrical nerve stimulation (TENS) and therapeutic ultrasound (US) and patients in the manual therapy group received manual techniques for the median nerve and its surrounding structures. Interventions were applied 3 times weekly for 4 weeks in both groups. Visual analogue scale (VAS), symptom severity scale (SSS), functional status scale (FSS) and median neurodynamic test (MNT) were evaluated before and after the interventions in both groups. Paired t-test and independent t-test were used for statistical analysis. Results Paired t-test revealed that all of the outcome measures had a significant change in the manual therapy group, whereas only the VAS and SSS changed significantly in the modality group at the end of 4 weeks. Independent t-test showed that the variables of SSS, FSS and MNT in the manual therapy group improved significantly greater than the modality group. Conclusions Manual therapy techniques applied to mechanical interface of the median nerve and nerve mobilization possess more appropriate and valuable effects on hand difficulties than modalities in diabetic patients with CTS.

Abstract: Introduction : The aim of this study was to compare the diagnostic value of cone-beam computed tomography (CBCT) and periapical (PA) radiography in detecting internal root resorption. Methods and Materials : Eighty single rooted human teeth with visible pulps in PA radiography were split mesiodistally along the coronal plane. Internal resorption like lesions were created in three areas (cervical, middle and apical) in labial wall of the canals in different diameters. PA radiography and CBCT images were taken from each tooth. Two observers examined the radiographs and CBCT images to evaluate the presence of resorption cavities. The data were statistically analyzed and degree of agreement was calculated using Cohen’s kappa (k) values. Results : The mean±SD of agreement coefficient of kappa between the two observers of the CBCT images was calculated to be 0.681±0.047. The coefficients for the direct, mesial and distal PA radiography were 0.405±0.059, 0.421±0.060 and 0.432±0.056, respectively ( P =0.001). The differences in the diagnostic accuracy of resorption of different sizes were statistically significant ( P <0.05); however, the PA radiography and CBCT, had no statistically significant differences in detection of internal resorption lesions in the cervical, middle and apical regions. Conclusion: Though, CBCT has a higher sensitivity, specificity, positive predictive value and negative predictive value in comparison with conventional radiography, this difference was not significant. Abstract: Introduction: Wound infection is one of the most common complications of surgical procedures. At present, different procedures are used to reduce wound infection including prophylactic antibiotics. Since laparoscopy controls the most sources of wound infection such as mechanical factors, the role of prophylactic antibiotic therapy may be in doubt. In this study, we evaluated the antibiotic effects in prevention of wound infection in laparoscopic cholecystectomy. Methods and Materials: In this double-blind clinical trial study, patients who were candidate for laparoscopic cholecystectomy were divided randomly into two groups: antibiotic receivers (38 patients) and placebo (Normal Saline) receivers (32 patients) group. The patients were visited in 24 hours, 4-7, 10-15 and 30 days after surgery for wound infection and then 2 groups compared. Results: The study showed that there were no significant differences between two groups with respect to age, body mass index and smoking. In addition, no wound infection was observed in first 24 hours, 4-7, 10-15 and 30 days after operation in both groups. Conclusion: The findings showed that prophylactic antibiotic therapy in laparoscopic cholecystectomy has no effect on the incidence of wound infection. Therefore, it is suggested that prophylactic antibiotic therapy does not use in laparoscopic cholecystectomy because of producing antibiotic resistance, unnecessary complications and also reducing economical costs Keywords: Wound infection, Cholecystectomy, Prophylactic antibiotic, Laparoscopy
