TL;DR: The results suggest that the depressed expression of the Klotho-FGFR1 complex in hyperplastic glands underlies the pathogenesis of SHPT and its resistance to extremely high FGF23 levels in uremic patients.

TL;DR: In this article, the authors investigated the effect of PTx on survival in maintenance hemodialysis patients with severe secondary hyperparathyroidism (SHPT) and found that successful PTx may reduce the risk for all-cause and cardiovascular mortality in patients with uncontrolled SHPT.

FGF23-parathyroid interaction: implications in chronic kidney disease.
TL;DR: Recent insights into the role of FGF23 in mineral homeostasis are summarized and the involvement of its direct and indirect interaction with the parathyroid gland is discussed, particularly focusing on the pathophysiology of secondary hyperparathyroidism in chronic kidney disease.
